Please complete and return this entry form and a completed roster,a $400 Mite Major & Minor, $300 A & B or $200 Pony’s deposit by Friday January 22nd. If we do not receive the roster by this date the players will not make it into the tournament program.
We will also require a current league score sheet from an already played game, with player’s and alternate’s names and numbers.  A copy of the team’s USA Hockey Roster as well as the remaining balance must be turned in by Friday January 29th.
